Homeschool Enrichment Co-Op Registration Form

This enrollment is for one full school year of classes, August 2025 – May 2026. It guarantees a spot in the class when class is being held, and not a set number of classes. Class will be held each week apart from normal holidays from 9am-12pm starting August 14th, 2025. Our daily schedule can be seen above, and our holiday schedule is on our website each semester. All students must pay a non-refundable $75 supply fee for the year. This includes books and supplies for Art and Spanish classes. The supply fee is due with the registration form, and tuition will be applied based on your selection below. Tuition can be paid monthly, semesterly, or yearly for the biggest discount. Once the registration is submitted the student will be an official member of class and subject to paying the dues to completion.
Holidays are listed on our school calendar each semester. There will be a dedicated make-up day provided for classes missed due to a declared weather emergency. The Octave will notify you if the classes will be canceled due to such an event. If Jefferson Parish closes their schools we will likely be closed as well.
We know that sickness happens, but due to the nature of this class we cannot offer individual make-up classes. Please try to be there as often as possible so that the class can proceed as scheduled.
All payments are non-refundable. Any withdrawals after class has begun on August 14th will be required to pay tuition. Please remember that we will reserve a space for you at the exclusion of other students.
There are two school recitals each year in December and May. The choir will be opening both of these events, and potentially others throughout the year. Information about performances will be emailed as soon as the dates are locked in. Each choir member is required to be at both of these events (health permitting). All music given in choir binders is property of the school and must be returned at the end of each year. Each student will be given a choir number to aid in reclaiming the music for future choir members. In the event of music being lost or destroyed please let Mr. Jay know as it will need to be reimbursed before the year is over.

All payments will done over the phone through the normal school payment processor after the form has been submitted.
